Guidance & Coordination

Faith Palliative Care is here to guide patients through the journey of their disease. We not only help individuals understand more about their disease, but also help them come to understand what their diagnosis means to them. We educate people about the available palliative care interventions and work collaboratively to develop a care plan that addresses their most pressing challenges and optimizes their quality of life.

Palliative care focuses on each individual’s wants and wishes, taking into consideration their personal values, beliefs, and goals. Our team is able to treat a variety of symptoms, including:

A team for all your concerns.

Our physicians, physician assistants, and nurse practitioners are certified in the specialized field of palliative care. They also collaborate with primary care physicians and other members of your care team to ensure alignment of care and goals.

Our palliative care team focuses on all your needs—emotional and psychological as well as physical. We can help you and your family better understand and cope with your condition. We can also help guide you through our often complex and confusing medical system.
